Medical Plans

  • Comprehensive Major Medical Plan
    Provides details about Comprehensive Major Medical (CMM), a Blue Cross Blue Shield of Michigan (BCBSM) health care program.


  • GradCare
    GradCare is for eligible graduate students. This book describes the benefits covered under the plan.

  • HAP Certificate of Coverage with Riders
    This booklet provides the provisions, limitations, exclusions and applicable riders of the HAP HMO benefit plan. The riders to the University’s agreement with HAP can be found at the end of the booklet.

HIPAA Notice of Privacy Practices and COBRA Rights and Responsibilities

  • HIPAA Notice of Privacy Practices and Notice of COBRA Rights and Responsibilities
    Privacy rules under the Health Insurance Portability and Accountability Act (HIPAA), apply to participants in certain types of health plans. The Consolidated Omnibus Budget Reconciliation Act of 1986 (COBRA) allows you to continue your coverage under certain health plans at your own expense after you terminate employment for any reason. The Benefits Office is required by HIPAA and COBRA legislation to provide these notices related to protections and privileges assured by these two federal laws.

Flexible Spending Accounts 

  • Flexible Spending Accounts
    This book describes Health Care Flexible Spending Accounts and Dependent Care Flexible Spending Accounts, which allow you to set aside pre-tax dollars for certain out-of-pocket expenses.

Dental Plan

  • MetLife Dental Plan (effective until 12/31/2008)
    Contains detailed information about the MetLife Option 1, Option 2, and Option 3 dental plans.


  • Delta Dental Plan (effective beginning 1/1/2009)
    Contains the certificate of coverage and plan information for Delta Dental Option 1, Option 2, and Option 3 dental plans.

    This summary provides general information about the legal plan.

Long-Term Disability Plans

Long-Term Disability Plans are designed to provide you with income and benefit protection should you become totally disabled.

  • Basic Long-Term Disability
    The Basic Long-Term Disability (LTD) Plan is available to AFSCME members only, after four years of service.

  • Expanded Long-Term Disability Plan
    The Expanded Long-Term Disability Plan is available to faculty (except Supplemental faculty), staff, and members of IUOE, POAM, MNA, and Trades.

  • U-M Same-Sex Domestic Partner Policy
    University of Michigan SSDP policy statement and resource information.

    Please note: U-M benefits for same-sex domestic partners (SSDP) concluded at the end of 2007 to comply with the current Michigan Court of Appeals ruling on same-sex partner benefits. Accordingly, references to U-M recognized Same-Sex Domestic Partnerships in all University policies and documents (other than existing collective bargaining agreements) have been rendered invalid.
