If you and your spouse get divorced, you must remove that person from your benefits by completing and submitting a Notice of COBRA Qualifying Event Form (PDF) and a copy of your divorce decree within 30 days of the divorce in order for any refund of insurance premiums
to occur and COBRA continuation information to
be sent to your former spouse. If you notify the HRRIS Benefits Transaction Team
of a divorce after 30 days but within 60 days, COBRA continuation
will still be sent; however, no refund of premium is available. The HRRIS Benefits Transaction Team can remove a former spouse after 60 days, however, no
COBRA continuation information will be sent and no refund of premiums
will be given.
Qualified Domestic Relations Order (QDRO)
You may also need to update:
- Your beneficiary designation. You must complete separate beneficiary designation forms for your life insurance plan and TIAA-CREF and Fidelity retirement savings plan investments.
- Your address, phone number, and emergency contact information. Go to Wolverine Access and select Employee Business.
- Your federal and state W-4 tax withholding information. Go to Wolverine Access and select Employee Business.
If you are eligible for benefits as a U-M employee and are covered as a dependent on your spouse's U-M plan but will lose that coverage due to divorce, you may enroll in benefits by completing and submitting a Benefits Enrollment/Change Form along with a copy of your divorce degree (be sure to include the signature page) within 30 days of the divorce.
