EVENT
WHAT TO DO WHEN TO DO IT
Confidentiality
Always maintain confidentiality of the employee’s health information. During the illness or injury, the recovery period, and thereafter.

Non Work-Related
Illness or Injury
Report the illness or injury to Work~Connections if:
  • It is known that the employee will be absent 10 days or more; or
  • After 10 consecutive days of absence or; if a chronic illness, after 10 intermittent days of absence in a three-month period; or
  • The employee requests it prior to the 10th consecutive day of absence.
Supervisor Completes the Report Form
Fax or telephone the completed form to Work~Connections. Fax (734) 936-1913.
On or before 10 consecutive days of absence.
Employee Completes the Medical Information Release Form
Provide to Work~Connections in person, by fax or by mail
Upon referral into the program.
All Medical Information will be coordinated through Work~Connections

Work-Related
Illness or Injury
Report Incident to Your Supervisor
If the employee requires medical treatment, supervisor will refer her or him to one of the University’s designated treatment facilities.

Note: If Emergency response is needed, call 911

Immediately
Report the illness or injury to Work~Connections. Fax or telephone the completed form Immediately (within 24 hours).

Recovery Period
Employee
Work~Connections will contact you for information.

Supervisor
Follow up with Work~Connections for updates throughout the employees recovery period.

Regularly, as needed.
Provide information as requested to the Work~Connections staff. Immediately upon request.

Return to Work
In cooperation with Work~Connections staff, the employee, supervisor, and treatment professionals, a return-to-work plan will be developed for the employee that includes any necessary accommodations. When the employee is nearing release by treatment professionals.
Interact with Work~Connections staff to implement a successful return-to-work plan. When the employee is released to return to work.
Follow up and monitor progress. When the employee returns to work.
