Where to go for Treatment

Work-Related Illness or Injury

All work-related illnesses and injuries in all departments at the University must be reported to Work~Connections immediately (within 24 hours).

Designated Treatment Facilities for Work-Related Illness or Injury Only
If you become ill or are injured at work, the University of Michigan will refer you to one of the designated treatment facilities listed below.

Non-Work-Related Illness or Injury

Reporting of non-work-related illnesses and injuries to Work~Connections is indicated as follows:

After 10 consecutive days of absence from work in the event of a non-work-related illness or injury (10 days is defined as 80 hours, pro-rated for part-time, or a total of 10 days of intermittent absences in a three-month period due to a chronic condition) your supervisor is responsible for reporting your illness or injury to Work~Connections.

Earlier Reporting
In certain circumstances, your supervisor or you may report your illness or injury to Work~Connections prior to 10 days. For example, reporting may be earlier (1) if it is known that you will be absent for 10 days or more, (2) if you might benefit from Work~Connections assistance during the first 10 days of absence, or (3) if you request that your illness or injury be reported immediately.

Cost of the Program
There is no cost to you when you use Work~Connections services.

To Report an Injury or Illness
Complete the “Illness and Injury Report Form”  and fax it to Work~Connections at 936-1913. Keep the original for your file.
