How do I change my name?
You’ll need to fill out and send in the Address/Personal Data Change (240KB pdf file) along with a copy of the official documentation showing your correct name (e.g. marriage license, social security card).

How do I submit a Flexible Spending Account (FSA) reimbursement claim? Mail or fax your claims directly to SHPS. Forms, addresses and fax numbers are available on the Benefits Office website.

My spouse is losing coverage. Can I add him/her to my benefits?
Yes, but you must report the change within 30 days of the loss of coverage. Fill out and return the Benefits Enrollment/Change Form (96KB pdf file) as well as the Group Medical Insurance Commercial Transfer Application (40KB pdf file).  If your spouse was a U of M employee, you do not need to fill out the Commercial Transfer Form, be sure to include your spouse's UMID on the Benefits Enrollment/Change Form.

How do I add a new spouse/baby to my benefits?
Fill out and return the Benefits Enrollment/Change Form (96KB pdf file) within 30 days of the event (include any necessary documentation listed on page 2 of the Change form).

What if I have not received my medical/prescription cards?
You should receive medical/prescription cards approximately 2-3 weeks after you return your Personal Enrollment Worksheet (PEW). If it has been longer than 3 weeks since you turned in your PEW, contact your Insurance Carrier to request new cards.

Is Service X covered under my medical plan?
Check the Medical Plan Coverage Comparison Chart (488KB pdf file).  If the service you desire is not listed in the chart, contact the Insurance Carrier as they have detailed coverage information. You can also use the Medical Plan Navigator to help learn more about your options.

I never received Dental cards.
MetLife Dental does not send out participant cards. When you go to the dentist for services, tell them you have MetLife Dental, the contract holder's (employee's) social security number, and the group number (Option 1–84460; Option 2–84461; Option 3--84462).

Questions about Leave policy and eligibility are handled through Recruiting and Employment Services. Contact the Employment Office at 734-764-6580. Questions pertaining to benefits while on Leave are handled through the Service Center.

Check this list of benefit books. If you don’t see what you need, send an E-mail to benefits.books@umich.edu and include in your message the name or description of the material you want, your name, address, and daytime phone number.
