Tilt the head back slightly (about 45 degrees). This will straighten the airway to the inspiratory airflow with the aerosol spray. If your chin is tilted down, or if you keep your head in the normal position, the airflow will be more turbulent, causing aerosol impaction on the back of the throat, reducing drug delivery to the smaller airways.
