There are 3 types of paperwork filled out by each employee that pertain to intellectual property rights and which require a signature: 1) Application for Employment Form, 2) Faculty Personnel Record Form, 3) Supplemental Appointment Information Form

Prior to 1978

Certify truthfulness of statements made. Agree to abide by all university rules and regulations.

Employee oath plus certification of truthfulness of statements

Employee oath, minority group identification, alien certificate.

From 1978 to May 1981

Same as above plus an authorization to contact previous employers was added to the certification of truthfulness.

Same as above plus an authorization to contact previous employers was added to the certification of truthfulness.

Employee oath, non citizen of USA info, Affirmative Action info.

From May 1981 to October 1983

Same as above plus Regents Bylaw 3.10 was added "if offered an appointment would you agree to the conditions stated above?"

Same as above plus Regents Bylaw 3.10 was added "if offered an appointment would you agree to the conditions stated above?"

Same as above

From October 1983 to July 1993

Changes:

"Adherence to this bylaw and all Regents Bylaws, as they are from time to time amended, is a condition of continuing employment at the University."

The sentence: "If offered an appointment, would agree to conditions stated above?" was changed to "If offered employment, will you adhere to the conditions stated above?"

Starting in 1990:

Changes:

"Regents Bylaws 3.10 stipulates the conditions governing the assignment of property rights to members of the University faculty and staff: Unless otherwise provided by action of the Regents."

Starting May 1997:

Changes:

Regents Bylaw 3.10 added to form. "I have read and understand the employee oath and Regents Bylaw 3.10 and agree to the conditions as stipulated in each."
