Ineligible Appointment
If at any time your appointment percentage decreases below
the minimum required for your job/career family, you will no longer
be eligible for University contributions toward the dental
plan. You may continue the coverage for up to 18 months
through COBRA, if you pay the entire
premium plus a 2% administrative fee, and elect coverage
within 60 days of the date of notification.
Retirees
and Survivors of Retirees
If
you are enrolled in the Dental Plan when you retire, you
can continue dental coverage after retirement. For more
information, view the section Continuation
of Benefits for Retirees and Survivors of Retirees.
Newly
Eligible for Benefits
When you move from an ineligible appointment to an appointment
eligible for benefits or become newly eligible for benefits,
please contact the HR/Payroll Service Center within 30 days
of your new appointment for benefits enrollment information.
After
your benefits elections are processed by the HRRIS Benefits
Transaction Team, you can confirm your benefits enrollments
in two ways:

You will receive a Confirmation Statement within 30 days
after you enroll that summarizes your benefits elections
and the monthly payroll deductions for each. If you do
not receive a Confirmation Statement within 30 days, contact
the HR/Payroll Service Center.

Review your paycheck stubs to make sure that the proper
benefit deductions are taken. Promptly report any necessary
corrections to the HR/Payroll Service Center.
Bargained-for
Staff
If your new appointment is as a GSI, GSSA, GSRA, or as a
Research Fellow, you will be automatically enrolled in Option
1.
Leave
of Absence or Layoff

Start of Leave or Layoff
When you begin a leave of absence or layoff, the type
of leave or layoff determines whether you continue to
receive University contributions during the leave or layoff
period. In order for benefits under the dental plan to
continue during your leave or layoff, you must pay the
appropriate premium each month. Information on continuing
this plan is available from the HR/Payroll Service Center.
Coverage will be cancelled for the duration of the leave
of absence or layoff if continuation forms and premiums
are not received, except during an FMLA leave.
- Return
From Leave or Layoff
If you return to a benefits eligible position, the HRRIS
Benefits Transaction Team will attempt to automatically
re-enroll you in benefits. After your benefits elections
are processed by the HRRIS Benefits Transaction Team,
you can confirm your benefits re-enrollments in two ways:

You will receive a Confirmation Statement within 30
days after you return to work that summarizes your benefits
elections and the monthly payroll deductions for each.
If you do not receive a Confirmation Statement within
30 days, contact the HR/Payroll Service Center.

Review
your paycheck stubs when you return to work to make sure
that the proper benefits deductions are taken. Promptly
report any necessary corrections to the HR/Payroll Service
Center.
Any changes in dependent status that occurred
during the leave of absence or layoff must be reported
within 30 days of your return, or, if coverage was continued
during the leave of absence or layoff, within 30 days
of the event.

If you return from a leave to a new appointment as a GSI,
GSSA, GSRA, or as a Research Fellow, you will be automatically
enrolled in Option 1.

The Dental Plan will automatically continue during
an FMLA leave unless you sign a waiver to cancel coverage.
If premiums are not paid monthly, a cumulative deduction
will be taken from your payroll check upon return
from leave.
