The Option 1 Fee Schedule, which is available by clicking
the link below, applies to Dental Plan Option 1 only. It
is a schedule of predetermined payments for Type B, C, and
D dental services covered under the Option 1 dental plan.
The fee schedule amount represents the maximum amount the
Plan will pay for covered services.

Using
the Search Feature
The Option 1 Fee Schedule is sorted by the American
Dental Association (ADA) code for each type of service.
To search for a specific service within the fee schedule,
follow these steps once you have opened the PDF:

Click the Find button
on the Acrobat
Reader toolbar or choose Find from the
Edit menu.

Enter the word to find in the Find What box.

Click Find. Acrobat Reader finds the
first occurrence of the word.
