Cancel Enrollment

Cancel SRA
Use this form to cancel your SRA contribution.  Simply indicate $0 as the contribution amount and check the “Cancel” box under Section 3.  If you are also enrolled in the Basic Retirement Plan, your 5% contribution and the U-M 10% match will continue and is not affected with this form.

Cancel Basic Retirement Plan
Use this form to cancel your 5% contribution and the U-M match to the Basic Retirement Plan.  SRA contributions will also be canceled.

Cancel 457(b) Contributions
Use this form to cancel your contribution to the U-M 457(b) Deferred Compensation Plan.  Simply indicate $0 as the contribution amount and check the “Cancel” box under Section 3a.  Your request to cancel your 457(b) contribution will become effective no earlier than the month following receipt of your form.
