Benefits Enrollment Checklist

An online presentation on the benefits packages, as well as links to comprehensive information and forms necessary to enroll in the benefits plans, are available from the New Employees section.

Important: To obtain coverage for you and your eligible dependents, you must enroll in your benefits within the election deadline for your career/job family or bargaining unit. See Benefit Enrollment Deadlines and Defaults (PDF).
Your Benefits

This document contains a comprehensive description of the University's benefits programs.

You will receive a copy of this book when you attend New Employee Orientation (NEO). If you do not attend NEO, you can download the book from the Benefits Books page, or obtain a copy by calling the HR/Payroll Service Center, visiting the Benefits Office, or sending an email to benefits.books@umich.edu (include your name, work address, daytime telephone number, and the title of the publication you would like to receive).

Flexible Spending Authorization Form To enroll in a Health Care and/or Dependent Care Flexible Spending Account, you may enroll online using eBenefits or complete this form and return it to the HRRIS Benefits Transaction Team as indicated on the form.
LE/Dependent Group Term Life Insurance Application Form If you enroll your spouse or other qualified adult in life insurance, complete this form and a health statement and return them to the Benefits Transaction Team.
Beneficiary for Group Life Insurance Complete this form and return it to the Benefits Office to designate your life insurance beneficiaries.
Retirement Enrollment Kit

The Retirement Enrollment Kit contains information from the retirement investment companies—TIAA-CREF and Fidelity—retirement savings plan enrollment forms, a booklet entitled It's Your Retirement, and other information. To obtain a kit, call the HR/Payroll Service Center, visit the Benefits Office, or send an email to benefits.books@umich.edu (include your name, work address, daytime telephone number, and the title of the publication or kit you wish to receive).

To enroll in the retirement savings plan, review the information, make your selections, complete the application forms, and sign, date, and return them to the Benefits Office. Complete the Salary and Annuity Option Agreement Form and an application form for each retirement plan investment company you choose (TIAA-CREF and/or Fidelity).

Confirmation Statement A Confirmation Statement will be displayed online and emailed to you whenever you submit enrollment election on eBenefits.

However, if any provision on the benefits plans is unclear or ambiguous, the Benefits Office reserves the right to interpret the plan and resolve the problem. If any inconsistency exists between this site and the written plans or contracts, the actual provisions of each benefit plan will govern. The University in its sole discretion may modify, amend, or terminate the benefits provided with respect to any individual receiving benefits, including active employees, retirees, and their spouses, partners, and dependents.
