Order Your Medical ID Card(s)

At 5:00 p.m., Friday, February 7, 2003, the HR/Payroll Service Center took down the Web site that allowed you to report that you have not received a medical insurance ID card and to request a replacement.

With the exception of Blue Cross Blue Shield of Michigan/United of Omaha Major Medical plan participants, all faculty, staff, retirees, COBRA participants, and survivors who are enrolled in a University of Michigan medical insurance plan should have received a new medical insurance ID card for 2003 due to implementation of the new Prescription Drug Plan on January 1, 2003. Medical insurance plans mailed new ID cards from mid-December 2002 through mid-January 2003.

If you did not receive a new medical insurance ID card and have not yet ordered a replacement, you can order a card by calling the customer service number for the medical insurance plan in which you participate or, for the following plans, through their Web site.

M-CARE HMO, M-CARE Point-of-Service and GradCare

Health Alliance Plan (HAP)

To request a new card for Comprehensive Major Medical, a Blue Cross Blue Shield of Michigan plan, call the HR/Payroll Service Center at 734-615-2000 or 1-866-647-7657 (toll free).
