Submitting Reimbursement Claims

 

  • Mail or fax your claims directly to SHPS.
    • Mail your claim to SHPS at:
      SHPS FSA Processing Center
      P.O. Box 34740
      Louisville, KY 40232
    • Fax your claim to SHPS toll-free by calling 1-877-270-3922. The fax number dials into a document scanner so that the claim is immediately in SHPS system. Faculty and staff members faxing their claims may keep the originals.

    Claim forms are available on the SHPS's and Benefits Office Web sites.
    If you wish to enter the University of Michigan participant section on the SHPS web site, you will need to type the address (http://www.myshps.net) into your browser rather than link to their site from the Benefits site.
    SHPS provides more frequent reimbursement periods. Check your account balance and payment information on the Web or through SHPS's AccountLINK phone system. Typical claims processing takes ten business days.
  • Click on SHPS's Web site for an interactive worksheet to help you estimate your anticipated out-of-pocket expenses.
