Teaching questionnaires are designed on-line, individually or in groups, depending on the procedures established by individual departments. Some departments allow staff to design their own questionnaires for each individual class. Other departments generate all their questionnaires via a department request which is based on pre-defined core forms. Select your option from the choices below.

• Design an individual questionnaire on-line.You may design an individual questionnaire for one or more classes; the item catalog is also available on-line.
