Payment is due by mail after confirmation is received from the Gift Delivery Coordinator. Currently, we only accept check or money order. Please make check payable to "University of Michigan" and mail it to the address provided by the Gift Delivery Coordinator.

• For snack bags only: credit card number required (by phone)
• For balloons only: payment accepted by check after delivery is complete
• For snack bags & balloon combinations: credit card number required (by phone)

I authorize the University Unions staff to make any necessary changes to the items--using the closest comparable item available--in case the items are not in stock.  (This is rare, and we promise the order will be comparable even in the unlikely event that we have to make a substitution!)

This form is only a request; I understand that the order is not complete until the balloon staff of the gift delivery staff of University Unions receive it and send a confirmation of the charges.

I understand that I should place my order at least five business days before the requested delivery date for best results, and that orders placed within three business days of the requested delivery date may not be accepted.

*I understand that if I do not send full payment within two weeks, my student's university account may be charged the original price plus penalty fees of up to $10.00 per month.

I understand the following Cancellation Policy:  Upon confirmation of the order (sent by e-mail to the address listed on the form) and submission of payment by the requestor, the order is considered final and cannot be canceled.  In the event that the order has not yet been billed, and staff has not yet been assigned, cancellations will be considered.
