Please fill out the following questionnaire so that we can gather appropriate information about you and plan for an amazing trip together.  We are currently looking for some great people to fill many positions including: HEADers, HEADhosts, Sponsors, and HEADmasters.  Please take a look at the position descriptions to see what each of these entail.  Other questions can be directed to our email address or contact us personally via phone.

Below are important questions so we can plan and finance the trip accurately.  Please mark "MAYBE" if it is something you would be willing to do or work on.  Only mark "YES" if you are very confident that you agree with this statement.  Either way, please also add extra comments (places, quantity, etc) to your answers as much as possible.
