REGISTRATION

Registration for Michigan Rowing Summer Camp 2009

To register for camp, please fill out the online Camper Registration Form and mail payment so that we receive it within seven days of registering online. We are unable to accept online or credit card payments, and we are not able to guarantee a space for a camper for longer than the seven-day period.

• Complete and submit the Online Camper Registration Form HERE.

Registration is on a first-come, first-served basis. We do limit registration so that we may ensure a one-coach-per-boat ratio and lots of water time for each coxswain. If camp fills, we will begin a waitlist. In 2007 and 208, camp filled and we were able to accommodate only the first two people on the waitlist because of last-minute cancellations.

Checks should be made out to "Rothstein Rowing LLC." Please mail your check to Michigan Rowing Summer Camp 2009, Athletics Department, University of Michigan, 1000 South State Street, Ann Arbor, MI 48109. You will receive confirmation of registration within five days of receipt of payment.

PARTICIPATION

Participation Requirements for Michigan Rowing Summer Camp 2009

To participate in camp, please download, fill out, and submit the following forms. The University of Michigan requires that every Athletic Camp at Michigan, including our rowing camp, has these forms filled out completely and on file for each camper who participates. You are welcome to scan forms and email them to us, and then the camper may bring hard copies to on-site check-in on July 26th. If you prefer, you also are welcome to mail hard copies to us. Either way, please be sure to submit all of the forms by July 1.

• Parent/Guardian Consent, Medical Release, and Release from Liability Agreement Form
  Download the form HERE.
Note: In addition to a parent or guardian, the camper also is required to sign this form.
• Health Insurance Information Sheet, with Emergency Information and Contacts
  Download the form HERE.
• Health Questionnaire
  Download the form HERE.

Notes: In 2007 and 2008, many people forgot to enter dates of campers' most recent tetanus shot and measles immunization. Please remember to include that information on the form, and also to have parent/guardian and the camper sign the form.
• Physical Examination Form
  Download the form HERE.

Notes: This form must be signed by the camper's physician. The University allows us to accept a physician-conducted and physician-signed physical examination form clearing a camper for athletic participation at her or his school instead of this form as long as it 1) is not sport-specific (e.g., says only that the camper is cleared to play basketball) or includes rowing, and 2) is dated July 1, 2008, or later. If you have any questions, please feel free to contact us.
• Housing Permission Form
  Download the form HERE.

Notes: Both the camper and a parent or guardian are required to sign and return the first page of this form. One half of one of the pages intentionally is left blank. This form is not required of commuter campers.
• Housing Registration Card
  Download the form HERE.

FEES

Fees for Resident and Commuter Campers

CANCELLATION
POLICY

Cancellation Policy

Your camp fee, less a non-refundable administrative fee of $75, will be refunded if you cancel no later than June 1, 2009. If you cancel between June 1 and July 10, 50% of your camp fee will be refunded. A refund, less a non-refundable administrative fee of $75, will be granted for medical reasons at any time prior to the start of camp if the request is accompanied by a signed medical statement from the camper's physician declaring the camper unable to participate in camp activities. Campers may not participate in camp without having submitted all forms by the time of Sunday's on-site registration. If a camper is unable to participate because of missing forms, we are unable to grant a refund. No refunds will be given after the start of camp.
