dash, tip, memtest

The bootcd build scripts contained some tarballs containing
binaries.  That's obviously a hack, so I've created some
transcripts and notes for those pieces.

I added a few new transcripts to sixthday today:
	mdw-dash.T	- same contents as "dash.tar" in bootcd notes.
	lfs-dash.T	- updated copy of dash
	lfs-memtest.T	- updated copy of memtest (memtest86+ v1.11)
	lfs-tip.T	- tip from openbsd 3.3
I've also created build notes on terminator for:
	dash
	memtest
	tip
